A channel separate from complaints: this is where you report conduct — by FVR™ staff, an auditor, an examiner, or any member of the body — that compromises the impartiality or integrity of a certification.
A report flags conduct: an undisclosed conflict of interest, bribery, undue pressure on a certification decision, or any fact that compromises FVR™'s impartiality or that of an evaluator.
If your case is instead a dissatisfaction with the service, the treatment you received, or you want to appeal a certification decision, that's the complaints and appeals channel.
The case is investigated by the Impartiality Safeguard Committee, and FVR™ takes no discriminatory or retaliatory action against anyone who reports in good faith.
This form is not anonymous: to investigate and follow up we need to be able to identify the report. If you'd rather not leave your email, your case is investigated all the same, but we won't be able to tell you the outcome.
FVR™'s only reporting channel is the confidential email support@fvr.mx. This form sends your report to that same inbox with the subject «DENUNCIA», so the Impartiality Safeguard Committee can route it and open the corresponding case.
Describe the conduct you're reporting in as much detail as possible: who, what, when, and any evidence you have.
